Assessments

Care needs can be simple or complex. One may need support with activities of daily living due to a medical event, support with instrumental activities of daily living due to a cognitive impairment, or just a change in surroundings to accommodate a change in mobility. Smith Integrated Care Services is versed in a variety assessments and can help to identify what is needed using comprehensive assessments and care planning.

Our care managers are experienced and eager to assess for needs in a variety of settings, including what care is needed and how that has changed over time. We can also complete a home safety assessment, providing you with suggestions as to how to reduce fall risk. We will see any changes through to completion, including referring for specialists to make repairs and helping you access programs to make home modifications as quickly and inexpensively as possible.
